2024-2025 Vaccine Requirements

Connecticut General Statutes (C.G.S.), State Department of Education, and Weston School Policy mandate that all new/re-enrolling students, those entering Preschool/Kindergartengrade 7, and grade 9 are required to present a completed State of Connecticut Early Childhood Health Assessment Record/Health Assessment Record. State law requires complete primary immunizations and a health assessment, performed within the last year, by a legally qualified practitioner of medicine: a doctor, an advanced practice registered nurse or registered nurse, a physician assistant, or a school medical advisor prior to school entrance in Connecticut. All questions asked on the form are required including hemoglobin/hematocrit results as well as tuberculosis risk screening. This may include a skin test or chest x-ray (as determined by the student's personal physician) if a student is found to be at high risk for tuberculosis.
 
Listed below are the vaccination requirements in the State of Connecticut for entry in the 2024-2025 school year. Pursuant to Public Act 21-6, in order to be eligible for an exemption on religious grounds, a student must have been enrolled in school in Grades K-12 on or before April 28, 2021, and must have submitted a valid religious exemption form before that date. A valid Student Medical Exemption Certificate for Required Immunizations form must be filled out by a student’s medical practitioner and provided to the school nurse before entrance if they do not meet state vaccination requirements.

Students who are new/re-enrolling to the district, entering Kindergarten, grade 7, or grade 9 must provide the State of Connecticut, Department of Education, Health Assessment Record form (blue). The information in Part 1 must be completed by the parent/guardian. Part 2 and 3 is the Medical Evaluation/Immunization Record to be completed and signed by your physician. If your child is enrolled in a preschool program, the CT Early Childhood Health Assessment form is used (yellow). These forms are available online at CT.gov. Completed assessments may be brought in or mailed to your child's school during the summer (Attention: Health Office).  All completed physical assessments must be returned by the first day of school and must be reviewed by the school nurse for compliance.
 
Except as provided in Board of Education Regulation 5131.31 (exemption from requirements), students who do not show proof of the required vaccinations will not be allowed to enter school in August 2024. A completed health assessment must also be provided. These requirements are to protect the safety of other students.
